Restaurant and advertiser forms prepare an email to mcc3mail@gmail.com. Sending that email depends on your mail app. We do not run a third-party form service on this site.

If Stripe Checkout is later enabled, Stripe will collect the information needed to complete that order, including a shipping address for USPS orders.
